Membership Fee: The Health & Wellness Membership Program requires a monthly fee of $99, plus prescription (Semaglutide/Trizeptatide) fees. Payment will be automatically processed each month using the payment method provided at enrollment.

Prescription Fees: In addition to the monthly membership fee, members are responsible for prescription fees in the amount of $395 per vial of Semaglutide or $450-$790 per vial of Tirzepatide. Esteem Medspa will be responsible for writing, ordering, and fulfilling of each prescription shipped overnight to your door. No refunds permitted on prescriptions once ordered.

Injection Fees: For clients who don't want to administer their own injections, Esteem Medspa providers will inject for a $35 per injection fee, not included in your membership.

Eligibility: Membership of the Health & Wellness Program is subject to approval by our medical team. Certain medical conditions or contraindications may affect eligibility for enrollment. The FDA’s body mass index (BMI) guidelines also determine eligibility for our program. Those not within these ranges may not qualify. When unsure, don’t hesitate to contact us.

Eligibility BMI range: BMI must exceed 30, or you must have a BMI over 25 with a weight-related health condition to qualify.

Labs/Bloodwork: Prior to enrollment in the Health & Wellness Membership Program, members are required to complete labs (Hgb A1C, Lipid Panel, THS, & CMP) within the past 6 months or pior to their first appointment at Esteem Medspa. These tests will be used to assess the member’s current health status and ensure that Semaglutide/Tirzepatide therapy is suitable for them. The results will be reviewed by our medical providers to determine eligibility for enrollment.

The total estimated cost for these labs are $187 and can be completed as a walk in through your local Labcop. If a recent Hgb A1C, Lipid Panel, TSH & CMP has been performed in the last 6 months by your healthcare provider then our medical staff will accept these for review instead.

Estimated Labcorp costs:

-Hgb A1C & Cholesterol Test: $89

-Thyroid Stimulating Hormone (TSH): $49

-Comprehensive Metabolic Panel (CMP): $49

Additionally, members are required to undergo lab work once per year after enrollment in the program. This annual assessment allows our medical team to monitor the member’s progress, evaluate the effectiveness of the treatment plan, make any necessary adjustments, and ensure safety for optimal outcomes.

Please note that the cost of labs and bloodwork is not included in the membership. All expenses related to bloodwork and labs is the responsibility of the client.

Cancellation Policy: Members may cancel their membership at any time by providing written notice to Esteem Medspa at least 30 days before the next billing cycle. No refunds will be provided for partial months. No refunds are permitted on prescriptions once ordered.

Results may vary depending on various factors, including but not limited to, adherence to treatment plans, medical history, and lifestyle choices.
